You have received this letter as a friend or family has completed an Authorization for Release of Information as you are someone that may need to speak with his/her counselor and/or come to visit the client. In order to speak with someone about the client, you will need to provide the number listed above along with information about you and the client currently in treatment. Additionally you will need to provide this letter when you come to visit along with photo identification. Please only bring this letter and your ID when you arrive; purses, bags, diaper bags, etc are not permitted during visitation. Please visit only in the location provided, clients may not accompany you to your vehicle before you leave.
Please do not bring any items to the client that the client cannot have in treatment such as: alcohol, drugs, paraphernalia, cell phones, media player, or other valuables. Items brought in for the client must be surrendered immediately for proper search, so please make sure the item and/or bag is clearly marked.
Beginning on March 13, 2005 any visitor must complete the mandatory 1 ½ hour Family Al-Anon Session before visiting with any client. The session will begin at 1 p.m. and end at 2:30 p.m. and you may visit the client afterward until 4:00 p.m.----- unless it is his first week of treatment, as Buffalo Valley does not permit visitation the clients’ first week of treatment. (Please note that visitation is from 1:00 p.m. until 4:00 p.m. on Sunday only—Saturdays in Clarksville.)
If you do not attend this session, you will not be permitted to visit with the client. After you complete the mandatory Family Session, you are not required to complete it again. It is imperative that you be on time for the session. ***If you are late, this may interfere with your visitation privileges.
Clients shall be directly involved in all treatment plan development. When indicated, the client’s family and/or significant other are also included. Please contact Buffalo Valley during business hours regarding issues that arise during client stay, including coordination of services, safety, grievance and ethics reporting.
